Instructions for Institutions / Hospitals to fill Online Application to Start/Renew CPS Courses for Academic Year 2024
1) Before filling up the online application form to Start/Renew CPS Courses for Academic Year 2024, Institutes / Hospitals are advised to carefully go through the Relevant Notification available on CPS website https://www.cpsmumbai.org.
2) It is mandatory to fill online application by all aspirant Institutions / Hospitals to Start/Renew CPS Courses for Academic Year 2024 . Applications submitted on any other format will not be accepted.
3) Applicant cannot update/edit application once application Submit. If Application is not Submitted and wants to Update/Edit incomplete application, Please provide OTP received on your Mobile. Last date to Fill Application for is 31/10/2023. After this date, Link will be closed for New Application. Last Date to Print and Submit Physical Application form to CPS is 31/10/2023. After this date, No application will be able to Print.
4) Institution / Hospital need to fill application form in total 7 Staps viz

    Step-1 : General Information of Institute / Hospital,
    Step-2 : Information of Courses Apply,
    Step-3 : Information of Departments,
    Step-4 : Coursewise MSR,
    Step-5 : Coursewise Type of Investigation Information (Only for Radiology/Pathology/Micro-biology Department),
    Step-6 : Coursewise Faculty Information and
    Step-7 : Submit Application form, Print Various Parts of Report and Upload Application Form and other required documents.

Missing of any Step in application form will summarisely to reject application form.
5) After Step-1, a unique application number will be generated for each new application which will be further use to identify the application.
6) After completing Step-6, application can be print in 5 stages viz

    1) Print Instructions,
    2) Print Covering Letter with General Institute Information,
    3) Print Departmental Information,
    4) Print Unit Formation in the Subject and
    5) Print Faculty Form and its Declaration.

After printing of all 5 stages, application can be signed and stamped whenever required and upload all pages alongwith required documents (as mentioned in Annexure-I i.e. Covering Letter with General Institute Information) and payment paid details in single PDF file.
7) After online submission of application form, within 4 days payment should be done and within 7 days signed and stamped application alongwith required documents (as mentioned in Annexure-I i.e. Covering Letter with General Institute Information) and information of payment paid should reached to CPS, Mumbai. Late application will not considered for further process.
8) Online application submit does not mean affiliation granted by CPS, Mumbai.
9) Please ensure that all information provided is correct and accurate. Affiliation on false information and certificate will be ipso facto null and void and also appropriate action will be taken by CPS Council.
10) The original copy of the payment receipt should be attached along with printout of online application form and mandatory documents as mentioned in Annexure-I i.e. Covering Letter with General Institute Information. Bank charges are extra and will be payable by the Institute / Hospital only.
11) Canvassing in any form by Institute / Hospital will disqualify the application.
12) All legal disputes pertaining to Start/Renew CPS Courses for Academic Year 2024 shall fall within the jurisdiction of Mumbai High Courts only.
